High points after dental restoration



Filling is called as high if dentist allows too much of filling material to project from the tooth cavity. High dental filling is not an uncommon event. Before dental filling, local anesthesia is given to the patient. It is difficult for patient to precisely duplicate a normal jaw relation when the patient is numb.When a dentist put the colored paper in patient’s mouth and tell him to bite and asks him if the dental filling feels high, at that time patient may not be able to tell that whether the filling is high or not.
Patient will feel high filling contour when the effect of local anesthesia wears off. When patient will bite normally, something will be stuck. Tooth with high dental filling will become sensitive to hot and cold food stuffs and biting pressure.







Dental filling too high is one of the common reasons which lead to pain after dental filling.


High dental filling leads to moderate to severe pain on application of pressure on dental filling. Tooth is supported in bone by periodontal ligament. If dental filling is too high, then it can lead to tenderness of the ligaments.

 Ligaments get compressed more than usual in case of high dental fillings and causes symptomatic apical periodontitis or acute apical periodontitis.

 If dental filling is high, then periodontal ligament gets widened and will become red and inflamed and causes pain.   

When the tissues in our body are put through stresses, then the tissues get tender. If you work for a long time in sun, then the tissue of your body will be strained up. To heal the strained ligaments, extra blood will be pumped in your hands which will make your hands appear red and swollen. Same thing happens with periodontal ligament when it gets strained due to high points in dental restorations.

How to Stop Pain Due to High Dental Filling

To stop the pain due to high filling, Patient can call the dentist and tell him that dental filling is too high and can get the selective grinding procedure done. Dentist will place a colored paper (articulation paper) in patient’s mouth and will tell him to bite.
       With colored paper, high points will appear on the filled tooth. Dentist will grind the high filling and will recheck the bite and the procedure will take just few minutes. Dentist will smoothen down the filling with his hand piece and burs. Smoothening of filling is done so there is no requirement of anesthesia to numb the tooth.  Dentist will probably not charge for the procedure as it was dentist’s fault that he didn’t grind the filling and corrected the bite after dental filling.  
Pain and sensitivity for few days after dental filling is normal but if there are is tooth sensitivity on biting down, then patient should visit his dentist.

Problems with High Dental Filling

Dental filling too high can lead to following difficulties:
  1. Pain on biting down: Dental filling too high can lead to pain on biting down. With high dental filling, periodontal ligaments around tooth will become inflamed and will cause soreness when the patient bites down on tooth. If the bite is affected because of high dental filling, then other teeth may also get affected.
  2. Tooth ache and sensitivity: Pulpal inflammation may occur with high dental filling or the tooth may become sensitive to hot and cold food stuffs. 
  3. Tooth wear: With high dental filling, affected tooth wear can occur. If bite of tooth is affected, then along with the filled tooth, wear of other teeth may also take place. 
  4. Temporomandibular joint problem: With high filling in the tooth,Temperomandibular joint problem can occur as the bite is affected.
  5. Pain in muscles due to high dental filling: Because of high dental filling, jaw muscles can become sore and can cause pain. With high dental filling, bite is changed and the muscles get strained. Pain in muscles leads to difficulty in opening mouth and chewing food.
  6. Headaches: Pain in muscles along with the TMJ pain can lead to headaches and discomfort to the patient.
  7. Teeth mobility: With high dental filling, jaw shifts and because of it, other teeth may be subjected to high stresses which can lead to looseness of teeth.  
  8. If patient has high dental filling present and the tooth is sore, then he should visit his dentist and get it checked before the problem aggravates. 
In general patients should be advised to bite and check for himself if all teeth are clenching at once or the recently filled tooth is clenching first. Occlusal equilibrium is important.Recall the patient after 24 hours again and check again.
